ExxonMobil shared a drops near miss incident that occurred on the Noble Don Taylor drilling rig. A dropped object can have a devastating impact and even cause a fatality
What Happened?
While picking up 13-3/8” casing on the auxiliary rig floor, a 7/8” 4-part shackle connecting a secondary retention sling to the auxiliary drill line guide fell to the aft pipe skate catwalk.
The shackle weighed 2.6 lbs. and fell ~150 feet.
The nut, weighing 2.6 oz., was found on the motor shed ~85 feet below the drill line guide.
Nearest crew member ~25 feet away under a drill floor drop shed within the wind wall.
Casual Factors
Improper utilization of cotter pin in the shackle.
Preventative Measures
Annual Drops Inspection completed Sept. 2019
Third Party Drops Inspection completed Nov. 2019 as part of rig acceptance.
Weekly derrick inspections completed by crew onboard.
Standardized HARMS management plan implemented with strict adherence.Corrective Actions
Refresh crew on proper 4-part shackle and cotter pin utilization.
Conduct derrick survey to identify and remove equipment rarely or not used to eliminate excess equipment at height.
Begin replacing all cotter pins annually during Drops / NDT inspections.
Derrick and drops inspections to include photo illustrations moving to confirm equipment acceptable and cross reference against drops register which already has pictures includedThe photographs are the ones included in the ExxonMobil safety alert.
Together we can all make our operation safer, spend 15 minutes a shift checking for drops potentials, that 15 minutes could save a life!
